Very poor management nowadays. It's just one way communication. Employees are not seen as an asset. It is hardly seen that management is willing to invest on its own people nowadays. Poor (R&D) product quality control & product development management. Slow to react on the market/competition. Sometime, simple thing can be very complicated. Too bureaucratic. Too many managers, and less staffs who are really doing the real jobs. Out of scope job delegation coming from the top.
